It is especially difficult for seniors and those with disabilities, but help is available. The Prince Edward Family Health Team’s System Navigator, Lee Marchildon, helps patients and service providers find what they need, coordinate services, and resolve communication problems. She is also involved in community outreach and education. In Part 1 (of 2 interviews) we learn how Lee and PEFHT can help you connect with the healthcare services you need.
